What do you need to register your child at Goodyear? 

There are certain documents required for any child to be enrolled in a Georgia school.  According to this law, your child must have all the records listed below on file at school. 

A copy of a Certified Birth Certificate. 
This is a copy with a state file number.  You can get a copy from the Vital Statistics office at the Health Department on Fourth Street, Monday-Friday from 8:00 – 5:00.

A Georgia Certificate of Immunization (Form 3231).
Immunization certificates are issued by the Health Department or your health care provider.

A Georgia Eye, Ear, Dental Form (Form 3300). 
This form may be obtained from the Health Department or your health care provider. It must be dated within a year of the first day of school.

A copy of your child’s Social Security Card. 
Those objecting to the use of the social security number may sign a waiver provided by the school office.
